Clinical Pharmacist

Overall Purpose of the Job:
The Clinical Pharmacist will be responsible for ensuring optimal use of medication and addressing any medication-related issues in line with hospital protocols and patient care objectives.
Roles and Responsibilities
- Prescription and Treatment Review: Verify prescriptions and ICU treatment sheets for accuracy, appropriateness, and dosage. Consult prescribing doctors where necessary to ensure the rational use of medicines.
- Therapeutic Drug Monitoring: Report on therapeutic drug levels for medicines with a narrow therapeutic index and make dose recommendations.
- Committee Participation: Serve on the Medicines and Therapeutics Committee, Infection Control Committee, and ICU Users Committee to enhance hospital-wide medication practices.
- Policy Formulation: Participate in the development of internal hospital policies relating to infection control, chemotherapy, antibiotic use, and disinfection.
- Prescribing Documentation: Assist in the design and continuous improvement of prescribing templates and documentation tools.
- Pharmaceutical Support: Provide accurate and timely drug information to doctors and consultants; support nurses and clinicians with pharmaceutical care.
- Clinical Partnerships: Collaborate with doctors for individualized patient treatment plans, especially in complex clinical cases such as renal management.
- System Updates: Ensure the pharmacy system reflects accurate stock levels and drug details.
- Patient Guidance: Counsel patients on correct dosage and proper use of prescribed medications.
- Departmental Reporting: Run and analyze statistics for pharmacy performance and patient outcomes.
- Committee Contributions: Represent the pharmacy in multidisciplinary committees to advance the hospital’s pharmaceutical knowledge base.
- Medication Interpretation: Interpret medication orders that require laboratory assessments to guide safe administration.
- Drug Vetting: Vet and register medications for hospital approval prior to dispensation.
- Error Monitoring: Track medication errors, analyze root causes, and implement corrective measures to improve workflow safety.
- Other Duties: Perform additional responsibilities as assigned by the supervisor, aligned with hospital goals and regulatory requirements.
Education and Experience
- Master’s Degree in Clinical Pharmacy from a recognized institution.
- Bachelor’s Degree in Pharmacy from a recognized university.
- Must be registered with the Pharmacy and Poisons Board of Kenya and possess a valid practicing license.
- A minimum of 3 years’ experience as a Clinical Pharmacist in a hospital setting.
