Position 1: Care Navigator

Reporting to: Assistant Manager, Care Navigation
Location: Head Office, Nairobi

Role Purpose

The Care Navigator will be responsible for guiding assigned members and schemes through their healthcare journey by offering professional advice on policy matters, ensuring access to quality and cost-effective care, and providing support in collaboration with healthcare providers, clients, and brokers.

Key Responsibilities

Strategy

  • Identify opportunities to improve program efficiency and patient outcomes.
  • Monitor regulatory developments affecting client care access.
  • Contribute insights for policy formulation aligned with care navigation objectives.
  • Analyze healthcare data to identify utilization trends and opportunities for service enhancement.
  • Use data to support decision-making and program development.

Operational

  • Track and analyze health-seeking behaviors of assigned members/schemes.
  • Enroll members into appropriate Wellness Programs.
  • Implement control measures based on utilization patterns.
  • Organize health education sessions for assigned schemes.
  • Monitor hospital admissions daily and coordinate member visits where necessary.
  • Offer condition-specific advice and direct members to appropriate levels of care.
  • Review medical reports to ensure alignment with treatment protocols.
  • Advocate for patient-centered care by engaging stakeholders on care-related concerns.
  • Vet healthcare services against coverage terms, treatment appropriateness, and cost.

Corporate Governance

  • Uphold ethical standards in all professional interactions.
  • Ensure compliance with healthcare, insurance, and privacy regulations.
  • Maintain detailed client records and care plans.
  • Clearly communicate care navigation services and limitations to clients.
  • Protect client confidentiality and seek proper consent before information disclosure.
  • Identify and mitigate risks associated with care navigation activities.

Key Skills and Competencies

  • Strong knowledge of healthcare systems, medical terminology, and insurance policies
  • Excellent communication, problem-solving, and active listening skills

Academic Qualifications

  • Degree or Diploma in Clinical Studies
  • Basic understanding of insurance concepts
  • Proficiency in Microsoft Office Suite

Relevant Experience

  • Minimum of 3 years relevant experience, with at least 2 years in a clinical setting

Position 2: Senior Wellness Coordinator, Population Health

Reporting to: Assistant Manager, Wellness
Location: Head Office, Nairobi

Role Purpose

The Senior Wellness Coordinator will lead the design, implementation, and evaluation of population health programs and wellness initiatives aimed at enhancing health outcomes and managing cost-effectiveness across member schemes.

Key Responsibilities

Strategy

  • Design and implement disease-specific interventions for various schemes.
  • Monitor health profiles and propose control strategies for population wellness.
  • Advise on health initiatives that reduce costs and improve care delivery.
  • Develop tools, indicators, and guidelines for disease surveillance at scheme level.
  • Manage and report data to support departmental insights and strategies.
  • Plan and execute quality improvement and disease management programs.
  • Identify cost drivers and design care navigation strategies accordingly.

Operational

  • Measure and report the impact of wellness programs.
  • Conduct real-time trend analysis using claims data and recommend interventions.
  • Collaborate with stakeholders across healthcare sectors for coordinated client care.

Leadership & Culture

  • Promote a culture of empathy, excellence, and ethics within the wellness team.
  • Drive quality improvement across care navigation and wellness services.

Corporate Governance

  • Maintain high ethical standards with clients and healthcare partners.
  • Ensure full compliance with healthcare regulations and privacy standards.
  • Keep comprehensive records of wellness interventions and client engagements.
  • Clearly explain service offerings, limitations, and costs to clients.
  • Preserve client confidentiality and obtain authorization before information sharing.
  • Assess and mitigate risks within wellness and population health programs.
