Position: Clinical Pharmacist

Department: Pharmacy

Location: Mater Misericordiae Hospital, Nairobi

Main Duties and Responsibilities

The Clinical Pharmacist will play a critical role in ensuring the rational, safe, and effective use of medication within the hospital. The responsibilities will include, but are not limited to:

1. Scrutinizing Prescriptions and Treatment Plans
The pharmacist will be responsible for reviewing patient treatment sheets and prescriptions to confirm the accuracy and appropriateness of prescribed drugs. This includes assessing dosing accuracy, potential drug interactions, and the suitability of the route of administration. The goal is to ensure optimal therapeutic outcomes for patients while minimizing the risk of adverse drug events.

2. Liaising with Prescribing Physicians
The role requires close collaboration with doctors to support rational drug use. This includes making recommendations for alternative medications, dose adjustments, or therapy changes when necessary to enhance treatment efficacy and safety.

3. Pharmacy Transactions and Billing
The pharmacist will ensure timely and accurate posting of all pharmacy-related transactions in the hospital’s system for billing purposes. Attention to detail is crucial to maintaining the integrity of patient billing and inventory records.

4. Stock Management and Replenishment
Maintaining optimal drug stock levels is an essential responsibility. The pharmacist will be required to monitor stock levels regularly and make timely orders to ensure the pharmacy is adequately stocked to meet patient needs without delays or stockouts.

5. Drug Information Resource
Serving as a reliable source of drug information for patients and medical staff is a core component of the role. The pharmacist will provide clear, evidence-based guidance on drug indications, dosages, contraindications, side effects, and interactions.

6. Handling Controlled Substances
The pharmacist will be responsible for the proper storage, inventory, and dispensing of narcotics and psychotropic substances in accordance with legal and institutional regulations.

7. Participation in Clinical Ward Rounds
Active involvement in ward rounds allows the pharmacist to contribute clinical insight into medication therapies. This interdisciplinary collaboration ensures that pharmacological considerations are integrated into overall patient care plans.

8. Patient Counselling
Pharmacists will provide detailed counselling to patients, ensuring they understand how to take their medications correctly. Emphasis will be placed on compliance, side effects, and what to do in case of missed doses.

9. Involvement in Quality and Patient Safety Initiatives
The pharmacist will participate in the hospital’s quality management and patient safety programs. This includes documentation, analysis, and reporting aimed at improving the quality of services and minimizing medication errors.

10. Adverse Drug Reaction (ADR) and Medication Error Reporting
Documenting and reporting all adverse drug reactions and medication errors is an essential part of improving patient safety. The pharmacist will be involved in investigating incidents and recommending preventive measures.

11. Participation in Formulary Management
The pharmacist will receive, evaluate, and make informed recommendations on applications for the inclusion or deletion of drugs from the hospital formulary. These responsibilities will be carried out on behalf of the Medicines and Therapeutics Committee (MTC).

12. Drug Utilization Monitoring
Monitoring the use of medicines across the hospital is vital for controlling costs and improving patient outcomes. The pharmacist will generate quarterly and annual reports detailing drug usage trends and efficiency.

13. Clinical Evidence Interpretation
The pharmacist will interpret clinical data and evidence to develop and endorse drug protocols. This will be done in collaboration with medical teams to ensure treatment protocols are based on the most up-to-date and robust evidence.

14. Supporting Individual Patient Care Decisions
Beyond general protocols, the pharmacist will assess individual patient profiles, including comorbidities and existing therapies, to tailor medication plans accordingly.

15. Critical Appraisal and Clinical Governance
Using critical appraisal skills, the pharmacist will review clinical literature and cost-efficiency data to support clinical governance efforts. This ensures that medication choices are both clinically effective and financially sustainable.

16. Conflict Resolution in Medication Management
The role requires the ability to manage and reconcile differing professional opinions regarding medication use, aiming for consensus on the most beneficial therapy for the patient.

17. Participation in Committees and Working Groups
The pharmacist will be expected to contribute to various committees or working groups as requested. These may include policy review committees, drug safety panels, and interdisciplinary clinical forums.

18. Contribution to Departmental Quality Initiatives
The pharmacist will be expected to participate actively in quality improvement initiatives within the pharmacy department, contributing progress reports, audit results, and action plans.

19. Any Other Duties as Assigned
As a senior member of the pharmacy team, the pharmacist may be assigned other duties from time to time to support hospital-wide initiatives or respond to emerging challenges.

Required Qualifications for the Position

  • Bachelor of Science in Pharmacy (BPharm)
  • A Master’s degree in a healthcare-related field is considered an added advantage
  • Registration with the Pharmacy and Poisons Board (PPB)
  • Must hold a valid, current practising license
  • A minimum of three years’ post-internship experience in a clinical pharmacy setting
  • Strong attention to detail and a high degree of accuracy
  • Excellent communication and interpersonal skills
  • Supervisory experience
  • Proficiency in computer applications relevant to hospital pharmacy operations
