Position: Pharmacist
As a Pharmacist at Mater Misericordiae Hospital, the selected candidate will undertake a broad range of responsibilities critical to the safe and effective use of medications within the institution. This is a full-time position that requires active involvement in clinical care, pharmaceutical management, quality assurance, and staff education.
Main Duties and Responsibilities
The Pharmacist will be entrusted with the following key responsibilities:
- Review of Prescriptions and Treatment Sheets: Scrutinize patient treatment sheets and medical prescriptions to ensure accuracy, appropriateness, and compatibility of prescribed medications. This involves checking for correct dosage, drug interactions, therapeutic duplications, and correct route of administration.
- Clinical Consultation and Liaison: Regularly liaise with prescribing doctors and other healthcare professionals to promote rational and evidence-based use of medicines. Offer expert opinions and recommendations where adjustments in prescriptions are necessary.
- Pharmacy System Management: Ensure timely and accurate posting of pharmacy transactions in the hospital’s billing system. This is crucial for both accountability and efficient patient billing.
- Stock Management: Oversee timely procurement and replenishment of pharmaceutical stock to maintain optimal inventory levels. This includes working closely with procurement and inventory teams to avoid drug shortages or excesses.
- Drug Information and Education: Serve as the primary source of drug-related information for patients and clinical staff. Provide accurate and up-to-date advice regarding proper use, side effects, contraindications, and storage of medications.
- Regulated Substance Management: Responsible for the secure storage, accurate recording, and controlled dispensing of narcotic and psychotropic substances in compliance with legal and regulatory standards.
- Participation in Clinical Rounds: Actively participate in hospital ward rounds with the clinical team to offer pharmaceutical input on patient treatment plans and drug selection.
- Patient Counselling: Educate patients on prescribed drug regimens, particularly regarding adherence, side effects, food and drug interactions, and correct administration to improve therapeutic outcomes.
- Quality and Patient Safety Activities: Contribute to the hospital’s quality management systems and participate in patient safety initiatives. This includes documentation and analysis of pharmaceutical service outcomes.
- Reporting Adverse Drug Reactions (ADR) and Medication Errors: Monitor and report any adverse drug reactions or medication errors in line with internal protocols and national regulatory requirements. Participate in root-cause analysis and preventive strategies.
- Formulary Management: Evaluate and make evidence-based recommendations on drug inclusion or deletion in the hospital formulary. This is done on behalf of the Medicines and Therapeutics Committee (MTC).
- Utilization and Efficiency Reporting: Lead the development and dissemination of quarterly and annual reports on drug utilization within the hospital. These reports are critical for strategic planning and regulatory compliance.
- Evidence-Based Medicine and Protocol Development: Interpret clinical evidence to assist in the formulation of drug-use protocols and to make patient-specific therapeutic recommendations.
- Critical Appraisal and Clinical Governance: Apply critical appraisal techniques to review clinical and pharmacoeconomic data. This promotes sound governance and helps to assess the value of pharmaceutical interventions.
- Conflict Resolution in Clinical Settings: Mediate conflicting clinical opinions regarding drug use among healthcare professionals and foster consensus for the benefit of the patient.
- Committee and Working Group Participation: Represent the pharmacy department in various hospital working groups and committees as appointed, including MTC and quality assurance teams.
- Continuous Improvement and Reporting: Play a proactive role in quality improvement projects and submit timely reports on departmental performance and service improvement initiatives.
- Other Duties: Perform any additional responsibilities that may be assigned from time to time in line with the role and operational requirements.
Required Qualifications and Competencies
- A Bachelor of Science in Pharmacy (BPharm).
- Registration with the Pharmacy and Poisons Board (PPB) and possession of a valid current practising license.
- A minimum of three (3) years post-internship experience in a hospital or clinical pharmacy setting.
- Demonstrated attention to detail and commitment to accuracy.
- Excellent verbal and written communication skills suitable for multidisciplinary collaboration and patient education.
- Supervisory experience or ability to oversee pharmacy operations and junior staff.
- Proficiency in pharmacy-related computer applications and hospital information systems.
- A Master’s degree in a healthcare-related field will be an added advantage but is not mandatory.
