Main Duties and Responsibilities
The successful candidate will be responsible for the delivery of safe, effective, and patient-centered pharmaceutical care in accordance with the hospital’s mission and regulatory requirements. The roles span a wide range of clinical, technical, and administrative responsibilities within the pharmacy department. These include:
1. Clinical Evaluation and Review of Prescriptions
- Assess patient treatment sheets and prescriptions to ensure the appropriateness of prescribed medications.
- Evaluate for correct dosages, potential drug interactions, contraindications, and optimal routes of administration.
- Work closely with medical staff to verify and confirm prescriptions, especially in complex clinical cases.
2. Liaison with Prescribing Doctors
- Regularly consult and communicate with prescribing physicians to ensure rational and evidence-based medication use.
- Act as an advisory resource for medication-related queries from the medical and nursing teams.
3. Timely and Accurate Computer Transactions
- Ensure that all pharmacy-related transactions are accurately recorded in the hospital’s computer system.
- Guarantee timely data entry for billing and inventory tracking purposes.
4. Inventory and Stock Management
- Monitor inventory levels and initiate timely stock replenishment requests to maintain optimal stock levels.
- Coordinate with the procurement and stores teams to avoid stock-outs or overstocking.
5. Patient and Staff Drug Information Services
- Offer comprehensive drug information services to patients, physicians, nurses, and other healthcare providers.
- Provide updates on newly introduced medications, potential recalls, or drug discontinuations.
6. Legal Compliance in Handling Controlled Substances
- Oversee the secure storage, documentation, and dispensing of narcotics and psychotropic substances.
- Ensure adherence to the Pharmacy and Poisons Board and Ministry of Health regulations regarding controlled substances.
7. Participation in Clinical Ward Rounds
- Join the clinical team during ward rounds to provide pharmaceutical expertise.
- Review patient medications and recommend appropriate interventions or adjustments where necessary.
8. Patient Counseling and Adherence Support
- Offer individualized counseling to patients on medication usage, side effects, and adherence techniques.
- Follow up with patients when necessary to ensure continued compliance with prescribed regimens.
9. Quality Management and Patient Safety
- Take part in hospital-wide quality management systems focused on improving patient safety and clinical outcomes.
- Document, analyze, and report incidents, such as medication errors or near misses, as part of ongoing risk mitigation strategies.
10. Adverse Drug Reaction and Medication Error Reporting
- Lead in the identification, documentation, and reporting of adverse drug reactions.
- Develop strategies to prevent medication errors and provide education to clinical staff.
11. Formulary Management and Evaluation
- Receive and critically evaluate proposals for the inclusion or exclusion of drugs in the hospital’s formulary.
- Make informed recommendations to the Medicines and Therapeutics Committee (MTC).
12. Drug Utilization Reporting
- Monitor and analyze drug usage trends within the hospital.
- Produce quarterly and annual reports that inform planning, budgeting, and policy decisions.
13. Clinical Evidence and Protocol Interpretation
- Review and interpret clinical research to inform hospital drug protocols.
- Translate clinical evidence and patient-specific information into actionable pharmaceutical care plans.
14. Critical Appraisal and Governance
- Utilize critical appraisal skills to assess the clinical and cost-effectiveness of treatment options.
- Promote a culture of clinical governance and accountability in the use of medicines.
15. Multidisciplinary Collaboration
- Engage in multidisciplinary meetings and discussions to harmonize treatment plans.
- Reconcile divergent medical opinions to achieve the optimal therapeutic approach for patients.
16. Committee and Working Group Participation
- Represent the Pharmacy Department in various hospital committees and working groups.
- Contribute to institutional initiatives aimed at enhancing service delivery and patient safety.
17. Quality Improvement Initiatives
- Lead or participate in internal quality improvement projects targeting pharmacy services.
- Provide periodic progress reports and outcomes of improvement projects to management.
18. Other Duties
- Carry out any additional tasks that may be assigned by the Head of Pharmacy or Hospital Management, in line with the strategic goals of the hospital.
Required Qualifications and Experience
- Bachelor of Science degree in Pharmacy (BPharm).
- A Master’s degree in a healthcare-related field will be considered an added advantage.
- Registered and licensed by the Pharmacy and Poisons Board of Kenya.
- Possession of a valid and current practicing license is mandatory.
- Minimum of three years post-internship working experience in a hospital or clinical pharmacy setting.
- Demonstrated attention to detail and a strong commitment to accuracy in pharmaceutical care.
- Effective communication skills for interaction with patients and interdisciplinary teams.
- Proven supervisory and organizational skills.
- Competence in using computerized hospital and pharmacy systems.
