CHS is the principal recipient of a five-year tuberculosis grant under the “Tamatisha TB” project, currently in its first year. This project supports Kenya’s DNTLD across all 47 counties, with focused interventions in 16 counties that carry the highest TB burden.

Purpose of the Consultancy

CHS seeks to engage an experienced consultant or consultancy firm to carry out an in-depth Organizational Capacity Assessment (OCA) of the DNTLD. The objective is to strengthen DNTLD’s ability to implement Kenya’s 2024/25–2027/28 National Strategic Plan (NSP) for TB, Leprosy, and Lung Health through enhanced governance, operational efficiency, human resource development, financial sustainability, and partnership-building.

Background Context

Tuberculosis continues to be one of the deadliest infectious diseases globally. In 2023, an estimated 10.8 million individuals contracted TB, with 1.25 million deaths reported. Kenya remains among the top 20 high-burden TB and TB/HIV countries. In the same year, Kenya recorded around 124,000 TB cases, with only 78% treatment coverage. Drug-resistant TB (DR-TB) is a growing concern, with 706 cases reported and only 58% treatment coverage.

Children accounted for 13.3% of the total TB burden in the country. The ongoing strategic response emphasizes cutting-edge diagnostics, rapid tests, shorter treatment regimens like BPAL/M for MDR-TB, and four-month regimens for children, all aimed at improving detection and treatment success.

The DNTLD plays a central role in steering the national response, being responsible for policy development, technical leadership, coordination, monitoring, and evaluation of TB, leprosy, and lung health programs.

Rationale for the Organizational Capacity Assessment

An effective TB response hinges on strong leadership, systems, and capacity at the national level. The NSP calls for evidence-based planning and strategic resource allocation to close the gaps in care pathways. This OCA will examine the internal structures and functions of DNTLD to identify strengths and areas for improvement that would ensure the effective execution of the NSP. It will be a key input into future staffing, resource allocation, and donor engagement strategies.

Objectives of the Organizational Capacity Assessment

The primary objectives are as follows:

  1. To evaluate governance structures, leadership principles, and oversight mechanisms.
  2. To assess internal coordination, management processes, and decision-making frameworks.
  3. To review strategies for resource mobilization, partnership building, and advocacy.
  4. To assess implementation capacity, monitoring systems, and quality assurance mechanisms.
  5. To evaluate staffing structures, recruitment practices, and capacity-building systems.
  6. To examine financial planning, internal controls, and resource allocation systems.

Key Assessment Areas

  1. Governance and Oversight
    • Review of DNTLD’s leadership structures, governance framework, operational policies, succession planning, and accountability mechanisms.
    • Examination of mission, vision, and core values and their influence on strategic alignment and organizational performance.
  2. Organizational Management
    • Evaluation of strategic and operational plans.
    • Assessment of communication protocols, advocacy mechanisms, resource mobilization efforts, and stakeholder engagement strategies.
  3. TB Program Management
    • Review of program design, execution of planned activities, and the mechanisms in place for monitoring, evaluation, and continuous quality improvement.
    • Analysis of community involvement in TB interventions.
  4. Human Resources
    • Evaluation of staffing patterns aligned with the NSP.
    • Review of recruitment practices, position descriptions, staff supervision, mentorship, training, and capacity-building programs.
    • Assessment of management structures for volunteers and interns.
  5. Financial Management
    • Comprehensive review of financial policies and systems, internal control mechanisms, staff competencies, budgeting procedures, documentation, reporting frameworks, and long-term financial sustainability.

Scope of the Assignment

The consultant or consultancy firm will conduct the assessment at the national DNTLD level and in select counties. The focus will be on institutional leadership, operational capacity, human resources, infrastructure, service delivery, financing, and monitoring systems. The aim is to enhance both national and sub-national capacity to implement the NSP.

County-level visits will provide contextual insight into the integration and functionality of TB, leprosy, and lung health programs within decentralized systems.

Expected Deliverables

The consultant will be required to submit the following:

  1. Inception Report: Outlining the methodology, data collection tools, and implementation timeline.
  2. Draft Organizational Capacity Assessment Report: Detailed findings, analysis, and initial recommendations.
  3. Final Organizational Capacity Assessment Report, which must include:
    • Prioritized recommendations with an actionable implementation plan.
    • Proposed organogram and organizational strengthening strategy.
    • Partnership and stakeholder mapping.
    • Resource mobilization strategies.
    • Capacity-building roadmap.
  4. Executive Brief: A summary report suitable for donor and government advocacy.

Consultancy Timeline

The assignment is expected to be completed within two months from the contract signing, with a strict deadline for final deliverables set for September 25, 2025.

Required Qualifications

Applicants must demonstrate the following:

  • A Master’s degree in Public Health, Health Systems, Organizational Development, or a closely related field.
  • A minimum of 10 years of professional experience in TB programming, strategic planning, or health systems strengthening.
  • Proven experience in conducting organizational capacity assessments, preferably in the health sector.
  • Exceptional analytical, facilitation, and report-writing skills.
  • Familiarity with Kenya’s public health architecture, particularly TB and lung health programs.
