Job Description for Pharmacist

The Pharmacist at M.P. Shah Hospital will perform a wide range of duties that cut across patient care, clinical decision support, pharmaceutical operations, and educational initiatives within the hospital setting. The detailed job responsibilities include the following:

  1. Deliver direct patient care and clinical practice through decentralized and service-based programs. This involves identifying high-risk patients within the hospital setting and implementing interventions aimed at improving both the quality and safety of care delivered to these patients. By taking a proactive approach, the Pharmacist helps mitigate risks associated with medication use and promotes better therapeutic outcomes.
  2. Identify prescriptions that may present risks to patients and resolve potential issues before medication is dispensed. This responsibility requires the Pharmacist to carefully review and analyze all prescriptions, detecting errors, contraindications, or potential drug interactions. Such oversight ensures patient safety and prevents the occurrence of adverse drug events.
  3. Provide pharmaceutical services across the hospital. The Pharmacist will dispense prescription medications in strict accordance with the directions of physicians, consultants, and other healthcare professionals. In this role, the Pharmacist must maintain a high level of accuracy and ensure that every dispensed medication meets the required professional and ethical standards.
  4. Participate actively in pharmacy operations and the medication dispensing process. This will include collaboration with the procurement department to facilitate the timely acquisition, ordering, and dispensing of specialty medications. Efficient coordination in this area helps maintain adequate stock levels and prevents interruptions in patient care.
  5. Coordinate medication deliveries in collaboration with pharmacy managers. The Pharmacist will organize workflow and oversee activities relating to the distribution of medications throughout the hospital. This includes planning and ensuring the timely delivery of medications to the correct departments, wards, or treatment areas, all while maintaining efficiency and compliance with established processes.
  6. Supervise the prompt delivery of correct medications to patients in various categories, including inpatients, discharge patients, outpatients, and those attending satellite centers. The Pharmacist will ensure that all such processes are carried out within acceptable timelines, in alignment with hospital protocols, and in compliance with regulations set by the Pharmacy and Poisons Board.
  7. Facilitate experiential education and practice advancement by identifying and implementing improvements in the medication-use system. The Pharmacist will contribute to advancing patient safety, optimizing therapeutic outcomes, and controlling treatment-related costs by continuously improving the efficiency and safety of pharmaceutical practices.
  8. Promote patient awareness and understanding of medication regimens. This involves conducting comprehensive medication counseling sessions with patients. During these sessions, the Pharmacist will explain the treatment purpose, duration, dosage instructions, and possible side effects of medications. The goal is to ensure that patients have a complete and accurate understanding of how to use their medications safely and effectively.
