Job Description
The Health Records and Information Assistant will report to the Facility-In Charge under the technical supervision of the MEAL Manager. The role involves working closely with the Health, Nutrition, WPE, and Safe Programming programs to ensure efficient health data management that supports patient care, program monitoring, decision-making, and reporting.
Key Responsibilities
1. Data Management
- Maintain accurate, complete, and up-to-date health records for patients.
- Organize and store patient records following data privacy policies and facility protocols.
- Ensure timely entry of health data into systems such as KHIS, DHIS2, or LMIS.
2. Data Analysis and Reporting
- Extract, compile, and analyze data from EMR systems for monthly, quarterly, and annual reports.
- Track key performance indicators to support program monitoring and evaluation.
- Generate reports for facility leadership, donors, and government stakeholders.
3. Quality Assurance
- Conduct Data Quality Audits (DQA) with the MEAL team and Facility-In Charge.
- Ensure data accuracy, completeness, and reliability.
- Work with clinicians and MEAL teams to identify and resolve data discrepancies.
4. System Management
- Effectively manage both paper-based and electronic health records systems.
- Support the transition from manual to digital records systems as needed.
- Train staff on data tools and health information systems.
5. Collaboration and Capacity Building
- Provide mentorship and technical support to health facility staff on data management processes.
- Conduct training sessions for health staff and data clerks.
- Work with MEAL, clinical teams, and government stakeholders for data alignment and reporting.
6. Administrative Duties
- Manage medical records filing and retrieval systems.
- Ensure compliance with Ministry of Health (MoH) regulations and facility policies.
- Prepare and update facility whiteboards and reporting dashboards.
- Supervise incentive data clerks.
Required Qualifications & Experience
- Diploma or Bachelor’s degree in Health Records and Information Management or a related field.
- 2-3 years of experience working as a Health Records Assistant.
- Proficiency in KHIS, DHIS2, and other health systems.
- Experience working with Electronic Medical Records (EMR) is desirable.
- Strong skills in Microsoft Excel and data analysis tools.
- Knowledge of data management best practices and health indicators.
- Strong analytical, organizational, and interpersonal skills.
- Ability to train and mentor health facility staff.
