Reporting to the Deputy Chief Nursing Officer (DCNO), the Social Worker will play a crucial role in supporting patients and their families through the emotional, social, and financial challenges associated with illness, hospitalization, and recovery. This role requires a proactive, empathetic, and resourceful individual who can connect patients with essential community resources and facilitate discharge planning to ensure continuity of care.
Key Responsibilities
1. Psychosocial Assessments:
- Conduct comprehensive psychosocial assessments to evaluate patients’ emotional, social, and financial needs.
- Identify challenges that impact patients’ well-being and access to care during hospitalization and recovery.
2. Counseling and Support:
- Provide individual, family, and group counseling services to help patients and their families cope with illness and hospitalization.
- Offer crisis intervention and emotional support during critical medical situations or personal crises.
3. Care Plan Development:
- Collaborate with interdisciplinary healthcare teams to develop and implement personalized patient care plans.
- Facilitate end-of-life care discussions when necessary to ensure that patients’ wishes are respected.
4. Advocacy:
- Act as an advocate for patients, ensuring their rights are upheld and that they receive necessary services.
- Facilitate access to financial assistance programs, community resources, and support networks.
5. Discharge Planning:
- Assist in creating and implementing discharge plans that promote continuity of care post-hospitalization.
- Coordinate with community agencies, home health services, and rehabilitation centers to ensure patients receive appropriate follow-up care.
6. Communication Facilitation:
- Serve as a liaison between patients, families, and the healthcare team, ensuring clear and transparent communication.
- Provide guidance on treatment plans, available services, and patient concerns to promote patient-centered care.
7. Resource Referral:
- Connect patients and families with external resources, including financial aid programs, housing assistance, mental health services, and other community-based organizations.
8. Documentation:
- Maintain accurate and up-to-date patient records in compliance with healthcare regulations and hospital policies.
- Document assessments, counseling sessions, interventions, and outcomes in the patient’s electronic health record (EHR).
9. Education and Advocacy:
- Provide education to patients and families on healthcare options, resources, and decision-making.
- Assist patients in navigating complex healthcare systems, insurance processes, and legal matters such as guardianship and advanced directives.
10. Multidisciplinary Collaboration:
- Work closely with medical, nursing, and administrative staff to ensure comprehensive and coordinated patient care.
- Participate in team meetings, care rounds, and case reviews to integrate social work services within the broader healthcare framework.
