Job Objective:
The Care Manager will play a critical role in ensuring that healthcare services provided to patients are effective, efficient, cost-conscious, and in line with clinical and administrative best practices. This role also functions as a liaison between the hospital and third-party payers, thereby reinforcing financial sustainability, accountability, and excellent customer relations.
Key Responsibilities
The Care Manager at Avenue Healthcare – Kisumu Hospital will be entrusted with a wide array of responsibilities that touch on clinical oversight, insurance coordination, reporting, inter-departmental collaboration, and process optimization. The responsibilities include but are not limited to:
- Daily In-Patient Rounds:
Conduct comprehensive daily rounds for admitted beneficiaries from specific insurance schemes. Ensure timely admissions, accurate diagnoses, appropriate treatment plans, and adequate follow-up care. Provide relevant updates to both the hospital team and corporate clients. - Pre-Authorization Coordination:
Validate and issue authentic pre-authorizations in collaboration with the clinical medical team, ensuring that clients access medical services smoothly and ethically. - Corporate Reporting:
Prepare and analyze monthly reports for designated corporate clients. Reports should include trend analysis, market intelligence insights, and actionable recommendations to enhance business development efforts. - Internal Liaison:
Work closely with key internal departments—particularly credit control and clinical teams—to resolve billing and invoicing issues. Ensure financial documentation is accurate and transparent. - Case Review and Optimization:
Regularly assess the case management process to identify areas for improvement. Propose and implement strategies that enhance efficiency, effectiveness, and profitability. - Customer Experience Enhancement:
Develop and maintain positive working relationships with stakeholders such as insurance underwriters, agents, brokers, and individual policyholders to ensure seamless service delivery. - Data Analytics and Decision Support:
Source, compile, and interpret critical healthcare data. Provide management with the insights necessary for strategic and operational decision-making. - Preventative Care Program Support:
Assist in the development and implementation of preventative health initiatives such as wellness programs and chronic disease management. Facilitate health talks and patient education activities. - Operational Reporting:
Prepare routine business reports on a daily and/or weekly basis. These should provide insight into care quality, case volume, operational challenges, and other performance metrics. - Incident Documentation:
Ensure that all healthcare-related incidents are documented appropriately by staff. Facilitate regular discussions with departmental heads to address underlying issues and promote a culture of accountability. - Departmental Meetings and Care Coordination:
Attend all relevant interdepartmental meetings and convene fortnightly coordination forums. Identify gaps in patient care pathways and propose viable solutions for service improvement. - Collaborative Healthcare Delivery:
Engage and coordinate with consultants, clinicians, and other department leads to improve the overall quality and continuity of care. - Impact Assessments and Strategic Interventions:
Conduct regular impact assessments to evaluate the effectiveness of ongoing programs. Recommend the most suitable strategies to bridge care gaps and enhance service delivery. - New Staff Orientation:
Guide the onboarding of new personnel in departments that interface with care management. Ensure adherence to departmental and unit-specific procedure manuals. - Patient Satisfaction (NPS):
Work towards continuously improving the hospital’s Net Promoter Score (NPS) through proactive care management and quality assurance. - Additional Duties:
Perform any other responsibilities as assigned by the supervisor in accordance with the core functions of the Care Manager role.
Person Specification
Applicants for this role must possess the following qualifications and competencies to be considered:
- Must be a Kenya Registered Community Health Nurse or Kenya Registered Nurse / Midwife with at least three years of professional experience.
- Must hold a valid Kenyan nursing practice license.
- Up-to-date Basic Life Support (BLS) certification.
- Certificate in computer application skills from a recognized institution.
- A minimum of three years of proven experience working as a Care Manager or in a similar capacity.
- Demonstrated expertise in managing third-party payer systems and insurance policy coordination.
- In-depth knowledge of healthcare legal regulations and safety protocols.
- Solid experience in emergency care procedures and medical documentation.
- Strong interpersonal and communication skills for liaising with both internal staff and external stakeholders.
- Analytical mindset with excellent report-writing and problem-solving capabilities.
